* multidisciplinary team caring for patients from diverse cultural backgrounds across 15 ambulatory locations in southeastern Massachusetts * Telephone coverage 1-2 days / month * Outpatient * Schedule is 4 clinical days and 1 administrative day * EMR: Meditech
