The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care.One Team One MissionRole Summary:The Gary and Mary West PACE (GMWP) Social Worker is responsible for managing a caseload of participants working in close coordination with the GMWP Interdisciplinary Team (IDT). In addition to delivering standard social work services such as psychosocial assessments, care planning, counseling and case management, the Social Worker functions as a liaison between the participants, their family members, the IDT and other members of the participants. support network, as appropriate. Collaborates effectively with colleagues and stakeholders to promote the GMWP values, team culture, and mission.Position Summary:The Gary and Mary West PACE (GMWP) Clinic LVN is responsible for coordinating and providing direct and indirect nursing care to participants at the center as well as at participant's home, working in close coordination with the PACE Physician, Registered Nurse, clinic staff and other interdisciplinary team (IDT) members to manage ongoing care, complex clinical situations, and to support the operation of the GMWP primary care clinic.Essential Functions:Works under the supervision of the Clinic Director/Registered Nurse and maintains vocational nursing professional standards of care.Performs home visits to administer prescribed medications and treatments in accordance with nursing standards to supplement clinic services.Observes, records, and reports participant's condition and reaction to medication and treatments to physicians.Documents all interactions related to clinic visits in the electronic medical record.Instructs participants and family members regarding medications and treatment instructions in the home by CNAs, PCWs and homemakers.Keeps Home Care Supervisor and Interdisciplinary team informed on home care issues of participants.Attends morning meetings at center, family meetings, and any other meetings as assigned.Complete all required documentation in a timely and accurateProtect privacy and maintain confidentiality of all company procedures and information about employees, participants andFollow GMWP policies and procedures and participate in any required Quality Improvement activities, staff training andOther duties as assigned.Requires use of personal vehicle.Education and Certification:Graduate of an accredited school of nursing.Unencumbered California Licensed Vocational Nurse (LVN)License.Current Basic Life Support (BLS) / First Aid CPR Certification required.Current driver license.Valid auto insurance.Clean driving record.Skills & ExperienceMinimum of two years nursing experience in a clinical setting with a frail or elderly population.Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinicalHighly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergencyExcellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.Experience and competency working with people from diverse backgrounds.Commitment to unlocking the full potential of our most vulnerable seniors.Bilingual Spanish-English preferred.Medical Clearance:Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.Job offers are contingent upon a successful pre-employment drug screen, background check, and physical assessment.Gary and Mary West PACE provides equal employment opportunities (EEO) to all employees and applicants for employment without regard to race, color, religion, sex, national origin, pregnancy, military and veteran status, age, physical and mental disability, genetic characteristics, gender, gender identity, gender expression or sexual orientation or any other considerations made unlawful by applicable state or local laws.
