Salary: $5,720.00 per month Support Specialist - Summary

Responsible for implementing and documenting progress towards consumer goals, promoting self-reliance and independence. Supervise assigned roommates/companions. Ensure consumer protection, rights, grievance procedures, fair hearings, and incident reporting. Support Specialist - Essential Functions

The following are key responsibilities of the position: Ensure adequate staffing through scheduling, recruitment, hiring, training, and orientation of staff and volunteers. Supervise staff in consumer households. Provide 1:1 support in consumers' homes and communities as per Personal Plans. Support on 24-hour pager. Assist in the Circle of Support and Person-Centered Planning Process. Facilitate and develop the Circle of Support to build relationships with clients, families, friends, neighbors, and staff. Act as an advocate for consumers. Develop and expand consumer participation in community activities; schedule outreach rides. Complete required documentation, incident reports, case recordings, billing, and medication monitoring systems. Assist consumers in finding housing, roommates, and needed supports. Maintain client records, progress reports, and billing documentation. Maintain professional relationships with consumers, families, medical providers, and community members. Represent HOPE in the community. Act as a mandated abuse reporter. Perform other duties to ensure program efficiency. Qualifications Required

Bachelor's Degree in rehabilitation, social sciences, special education, health, public administration, social work, or related field, plus 3 years of relevant experience or equivalent combination of education and experience. Knowledge and Skills

Knowledge of community services Implement ISP objectives Knowledge of client rights and advocacy Understanding of housing markets and transportation Health & Safety considerations Environmental Conditions

Extensive driving, exposure to climatic conditions, occasional night and weekend work, lifting supplies, walking, standing, visual and auditory acuity, community-based work, and occasional stair climbing.
