This role involves providing essential rehabilitation services to patients in the comfort of their homes. As a Home Health PT, you will work one-on-one with a diverse patient population, including adults and geriatrics, to help them achieve their rehabilitation goals. Key Responsibilities: - Deliver personalized physical therapy services to patients in their homes, focusing on recovery and wellness.- Conduct thorough assessments and create tailored treatment plans based on patients' individual needs.- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.- Educate patients and their families on therapeutic exercises and home care strategies to support long-term recovery.- Maintain accurate and timely documentation of patient care and progress using the EPIC Remote Client for Home Health and Oasis tools. Qualifications: - Minimum of 1 year of skilled home health experience required.- Current Basic Life Support (BCLS) certification.- Familiarity with OASIS documentation is mandatory.- Strong communication and interpersonal skills to effectively interact with patients and their families.- Reliable transportation to travel to patient locations; occasional travel may extend up to a 60-mile radius. Desired Skills: - Experience using electronic charting systems, specifically EPIC.- Proficiency in provided patient care techniques, including but not limited to wound care, adaptive devices, and various therapeutic practices.- Ability to work independently and manage time effectively in a home health setting. Additional Information: - This position requires a flexible approach to patient care and the ability to drive to various points of service efficiently.- Weekends are not required, and there is no on-call duty.- Continuous professional development opportunities available.
