Essential Functions
  • Deliver direct nursing care to residents with empathy and professionalism.
  • Complete admission records and initiate personalized care plans.
  • Respond promptly to urgent and emergent situations.
  • Maintain a safe and supportive environment for residents.
  • Implement and monitor individualized care plans.
  • Supervise and mentor nursing assistants, ensuring high-quality resident care.
  • Document essential information accurately in medical records.
  • Monitor residents' health status and communicate effectively with supervisors.
  • Build and maintain therapeutic relationships with residents.
  • Participate in care conferences and ongoing staff development programs.
  • Utilize computer software for efficient documentation and communication.
  • Complete required computer education modules.
  • Adhere to corporate compliance policies, ensuring confidentiality and resident well-being.
  • Administers medications and treatments in accordance with established policies and procedures. Is familiar with indication for use, action, and major side effects of medications administered.
Education: Graduation from an approved registered professional nursing program.
Qualifications:
  • Active, current Illinois LPN license to practice professional nursing.
  • CPR Certification Required - Accredited by American Heart Association.
  • One year as an LPN in a hospital (general care) or long-term care setting with adults preferred.
  • Genuine interest in working with older adults.
  • Knowledge of aging changes and common health problems of geriatric patients.
  • Good communication skills.
  • Ability to direct nursing assistants.
