Purpose of Position
Under the direction of the Palliative Specialist and within the Agency policies and procedures, the LPN follows the plan of care on patients with a serious or life-limiting illness within the guidelines of his/her licensure to provide for the safety and quality of care treatment or services.
Description
  1. Provide palliative care services including symptom management, education on medications and lifestyle changes to enhance quality of life, while collaborating closely with palliative specialist and community providers.
  2. Work with MSW to initiate discussions on advanced directives and goals of care to ensure a smooth and timely transition to hospice.
  3. Assesses patient and family needs, reports new or pertinent findings to the Palliative Specialist RN and MD.
  4. Follows prescribed plan of care as completed by the RN in order to address identified needs.
  5. Demonstrates the skills and judgments to implement the medical plan of care, nursing interventions and procedures as necessary for the care of the patient, within the guidelines of his/her licensure.
  6. Evaluates and reassesses the identified problems, care provided and patient's response on a continuing basis.
  7. Identifies ancillary services needed by patients and families and reports findings to RN.
  8. Collaborates and communicates on a regular basis with the members of the inter-disciplinary team as needed.
  9. Assists patients and families in accessing appropriate community resources.
  10. Records narrative documentation to reflect skilled care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver.
  11. All other duties as assigned

Qualifications

  • Graduate of an approved LPN school of nursing.
  • Current LPN license in MA.
  • CPR certification required.
  • Minimum one (1) year recent experience in an acute care facility or the equivalent.
  • Homecare and computer skills preferred.
