Licensed Practical Nurse (LPN) - Case Management (Spanish Speaking Required)Fresh Meadows, Queens, NY$45 per hourA growing primary care and infusion center in Fresh Meadows is seeking a Licensed Practical Nurse (LPN) to serve as the lead case manager for our chronic and high-risk patient population. Fluency in Spanish is required to meet the needs of our largely Spanish-speaking community. This is a full-time position, Monday-Friday, 9 AM-5 PM. You will start in care coordination and telehealth case management; as infusion visit volume grows, part of your week will shift to hands-on infusion therapy (training provided).Key ResponsibilitiesPerform intake assessments, create and update individualized care plans, and coordinate services across providers and community resourcesConduct telehealth and in-clinic follow-up visits, monitor lab results, and track patient progress toward care-plan goalsServe as the primary point of contact for patients and families, ensuring clear communication in Spanish and EnglishArrange referrals, authorizations, and durable medical equipment; ensure accurate documentation in the EMREducate patients on chronic disease management, medications, and treatment adherenceTransition to administering IV infusions (start IVs, monitor patients, document therapy) once infusion demand increasesRequirementsActive New York State LPN licenseAt least 1 year of clinical experience; prior case management or care-coordination experience preferredFluency in Spanish (spoken and written) is mandatoryProficiency with EMR and telehealth platformsWillingness to learn and provide infusion therapy; prior IV experience is a plusOutstanding communication, organizational, and problem-solving skillsSchedule: Full-time, Monday-Friday, 9 AM-5 PMCompensation: $45 per hour, with growth opportunities as infusion services expand
