This position focuses on assessing patient health status, administering prescribed treatments, and monitoring progress to ensure optimal health outcomes. The LVN/LPN will collaborate closely with physicians, therapists, and family members to develop and implement individualized care plans tailored to each patient's unique needs. This role requires a compassionate approach, strong clinical skills, and the ability to work independently while managing multiple patient cases. Ultimately, the goal is to enhance patient comfort, promote recovery, and support long-term health management in a home setting.As a Home Health LVN, you will:Under the direction of the RN, assist in identifying the patient's physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.Assure the continuity of care through delivery of quality patient care.Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.Monitor assigned cases to ensure compliance with requirements of third party payor.Promote Agency philosophy and administrative policies to ensure quality of care.RequirementsCurrent LVN LicenseValid Driver's LicenseValid Auto InsuranceCPR CertificationHomeCare HomeBase experience preferredBenefits and PerksMedical, Dental, Vision Insurance401k with matchingHealth Savings AccountLife InsuranceShort & Long Term Disability InsurancePaid Time OffPaid HolidaysCompany vehicle assigned after 90 days upon availablity and all terms and conditions set by the fleet care team are met.
