This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse (RN). Services are furnished in accordance with Agency policies. In this role, you will report to the Director of Nursing (DON).Duties/Responsibilities:Furnishes services, treatments and diagnostic and preventive procedures as assigned per the plan of care, in accordance with agency policiesPrepares clinical and progress notesAssists the physician, the intermediate care provider and RN in performing specialized proceduresPrepares equipment and materials for treatments observing aseptic technique as requiredAssists the patient in learning appropriate self-care techniquesUnderstands and adheres to established policies and proceduresFollows/implements the nursing care plan for each patient as approved by the RN/physicianInitiates preventive and rehabilitative nursing procedures as appropriateObserves signs and symptoms and reports to the physician and RN nurse reactions to treatments, including drugs and changes in the patient's physical or emotional conditionTeaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at homeEvaluates with RN the effectiveness of the LPN's nursing service to the patient and family under the guidance of the RNMaintains accurate and complete records of observations, treatments and care of patientParticipates in medical record audit as assignedAttends staff meetings, patient care conferences and in-services as scheduledTakes on-call duty, nights, weekends and holidays as assignedSubmits any changes in schedule to appropriate scheduling position in their office on a daily basis or as applicable by EMR workflowParticipates in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech language pathologistObserves aseptic technique and standard precautions as requiredAccepts, writes and executes orders; orders are cosigned by RN where applicableEnsures that the patient/family is involved in implementing the plan of careAssists in reviewing, evaluating and updating the patient plan of care as appropriateAdministers medications and treatments as ordered; documents appropriatelyProvides appropriate information regarding patient to other agencies/ individuals involved in patient's care as needed and as directed by the RNObserves confidentiality and safeguards all patient related information (PHI) in accordance with HIPAA requirementsCompletes documentation and paperwork in a timely manner as per Agency policyImmediately reports/documents to Nursing Supervisor/RN any patient incidents/variances or complaintsDemonstrates competent performance of technical skills according to established procedures before performing new skillsParticipates in peer review, utilization review and QAPI activities as assignedProvide effective communication to patient/family, team members, and other health care professionalsUnderstands and adheres to established state, federal and/or accreditation policies/proceduresAdheres to Company standards and consistently interprets and accurately performs all assigned responsibilitiesMaintains acceptable attendance status, per Company policyReports all incomplete work assignments to Nursing Supervisor/RNAppearance is always within Agency standards; is clean and well groomedDemonstrates effective time management skills through daily documentation and infrequent overtime for routine assignmentsMaintains clean and neat work environmentDemonstrates sound judgment, critical thinking and decision makingPerforms other duties as assignedRequired Skills/Abilities/Knowledge:Average computer skills with competency in media communicationsMust have the physical abilities to perform the described dutiesAcceptance of vision, philosophy and goals of the CompanyAbility to exercise initiative and independent judgmentExcellent verbal and written communicationEducation/Experience/Licenses/Certifications:Graduate of an approved school of practical nursing and currently licensed in the state(s) in which practicingOne year experience in nursing (preferred)Active and valid CPR certification with practicumIV certification by state mandate, if required by the stateBenefits + Perks of Joining the Team Select FamilyMedical, Dental, and Vision Insurance401(k)Referral ProgramFlexible schedulingPaid, hands-on trainingWeekly payTuition Assistance Programs and Continuing Education opportunitiesNursing programs for new graduates available!Pay Range: $22.00 - $24.00 / hourWe accept applications on an ongoing basis.
