We believe in A ccess, V alue-Based Care, I nnovation, and D edication to Quality.Schedule: We are looking for an RN or LPN for a ** Monday - Wednesday night shift in the Kernersville Area **Essential Job Responsibilities: Evaluates and treats clients with appropriate level of skill according to client's needs under the supervision of an RN, Clinical Manager.Assist with providing continuity of care and emotional needs of client and family.Resourceful in meeting client's psychosocial needs.Verbal demonstration of client rights and responsibilities.Make decisions based on sound nursing judgements.Recognizes and copes with emergencies appropriately.Ensures proper management of client services.Demonstrates competent use of equipment.Appropriately communicates information relative to client care in accordance with agency policies.Observes the client's general physical, emotional, and mental status, and reports all changes in the client's conditions, family situation, incident, and occurrences during shift immediately to agency RN/CM or Customer Care Coordinator.Ensure all forms of communication with clients, families, and health plan providers are documented timely to promote continuity of care across all interdepartmental functions.Build relationships with clients to encourage compliance with care plans and client safety. Maintains accurate clinical and shift work documentations to update system in accordance with guidelines set forth by the agency in a comprehensive and timely manner. Adheres to infection control policies and exposure control plan as mandated. Monitor and maintain clinical report, including infection and Occurrence/Complaint Reports. Must have a current, unencumbered license as an LPN, or an Advanced Practice Nurse. Must be in good standing in the applicable state of practice. Must maintain all state and federal compliance requirements for licensure and employment.Travel: Travel is required for business purposes. Employees must have a valid driver's license issued by the state in which they work and a satisfactory driving record.Work Experience: Minimum of one year of home care experience. One additional year of nursing experience, preferably in a hospital, is required for conducting applicable training classes.Other Hours of Work: Varies on a case-by-case basis. Full time is at least 30 hours a week from Saturday-Friday.Skills and Additional Requirements:A specific understanding of geographical/cultural requirements of branch office is preferred. Must be customer service oriented and prepared to liaise with patients, their families and other care providers.Ability to use computerized systems.Satisfactory verbal and written communication skills.Ability to work well under pressure.Bi-lingual preferred
