Job Summary The PRN Social Worker with MSW plays a vital role in providing high-quality clinical social work services to patients and their families. This position is focused on enhancing patient outcomes, facilitating successful adjustment, and streamlining decision-making across the healthcare spectrum. Responsibilities include conducting comprehensive psychosocial assessments, delivering counseling and emotional support, coordinating care plans, and connecting individuals with necessary community and financial resources. Essential Functions Conducts psychosocial assessments tailored to the developmental needs of patients ranging from neonates to the elderly, addressing emotional, social, and financial circumstances. Offers empathetic counseling and educational resources to aid patients and families in coping with serious health challenges, disabilities, loss, and key life transitions. Engages in interdisciplinary discharge planning by identifying and addressing psychosocial barriers, ensuring effective transitions to skilled nursing, rehabilitation, or home care settings. Facilitates referrals to both internal services and community resources, including mental health providers, social service agencies, support groups, and financial assistance programs. Manages complex psychosocial cases, addressing high-risk referrals, such as new diagnoses or behavioral health concerns, and providing support in sensitive situations like abuse or end-of-life care. Provides crisis intervention to patients and families who are facing acute emotional distress, grief, or trauma. Collaborates efficiently with healthcare teams—including physicians, nurses, and case managers—to ensure seamless care planning and discharge processes. Contributes to quality improvement initiatives, focusing on readmission reduction and tracking psychosocial outcomes. Maintains accurate and timely documentation in medical records in compliance with regulatory and organizational guidelines. Acts as a resource for peers and may assist in mentoring or guiding less experienced social work staff. Performs additional duties as assigned to support the organization’s mission. Adheres to all organizational policies and industry standards. Qualifications Master's Degree in Social Work (MSW) from an accredited program by the Council on Social Work Education (CSWE) is required. Preferred: 2-4 years of clinical social work experience within a healthcare environment. Knowledge, Skills, and Abilities Strong understanding of social work principles, case management practices, and psychosocial assessment techniques. Demonstrated clinical judgment and counseling expertise to navigate complex emotional scenarios effectively. Familiarity with healthcare systems, post-acute services, and community resources. Exceptional verbal and written communication skills, particularly regarding electronic medical documentation. Able to work collaboratively in a dynamic and fast-paced interdisciplinary team. Exhibits empathy, cultural competence, and sensitivity towards diverse populations. Skilled at task prioritization, caseload management, and adapting to evolving patient requirements. Licenses and Certifications LMSW - Licensed Medical Social Worker - State Licensure is required. BCLS - Basic Life Support certification is required.
