Position Summary:

As a pioneer in accountable care programs for the elderly, we provide exciting opportunities across multiple states. If you are seeking greater flexibility beyond the conventional office setting, this role allows you to work in community-based Skilled Nursing and Assisted Living Facilities. Lead a dynamic team of Advanced Practice Providers and collaborate with an interdisciplinary team that includes psychiatrists, psychotherapists, podiatrists, and wound care specialists. Responsibilities: Deliver primary care services per Eventus WholeHealth protocols, in alignment with family, geriatric, and internal medicine standards for long-term care. Review and sign Medication Administration Records (MARs) as requested, along with other necessary orders within 24 hours. Address lab results promptly and manage scheduled medication prescriptions in accordance with our policies. Allocate sufficient time during each visit for paperwork and administrative duties. Participate in care plan meetings and Quality Assurance/Performance Improvement (QA/PI) meetings as required. Be available for on-call consultations between 8AM and 5PM on scheduled workdays, utilizing support from our ECare team. Submit required documentation and billing records according to our schedules. Foster positive communication with facility staff and administration, providing essential guidance on medications and caregiver information. Engage with the facility's administrator or designated care coordinator prior to starting and after completing daily patient care. Adhere to all Eventus WholeHealth policies, legal regulations, and ethical standards, maintaining patient confidentiality at all times. Qualifications: Licensed MD or DO required. A minimum of 3 years’ experience in Long Term Care or Geriatric Care preferred. AMDA Certification is a plus, though not mandatory. Strong leadership and organizational skills. Excellent written and verbal communication abilities. About Us:
