As a Licensed Practical Nurse (LPN) at Hi-Hope Service Center, you will support client health and wellness by performing nursing tasks, medication administration, care coordination, and clinical documentation under Georgia Medicaid Waiver guidelines. Key Responsibilities Complete assigned nursing care tasks and maintain timely, accurate documentationReview and manage CareSuite QuickMAR, Therap, and paper MARsAssist with medication administration and health care planningConduct basic triage and first aid when neededTransport residents to medical appointments as necessaryEnsure medical follow-ups and health risk tracking for assigned individualsParticipate in staff meetings and collaborative care planningRequirements Active LPN license in GeorgiaMinimum 1-2 years of experience in a clinical, residential, or ID/DD setting preferredProficient in Microsoft Office, EMAR systems, and electronic documentationMust be 21+, pass a physical exam, drug screen, background check, and TB screeningMust have a valid Georgia driver's license and reliable transportation
