Job Summary:
Under the general supervision of the Director of Patient Care Services/Director of Clinical, the Intake Coordinator plans, organizes, and assists in the coordination of services for patients/clients discharged from the hospital into the home health program. Assures appropriate identification of services in compliance with insurance parameters.
Duties and Responsibilities: Assist in the coordination of services for patients/clients discharged from the hospital into the home health program Assist in the development of the patient's/client's plan of care after discharge from the hospital Coordinate home care needs with the agency in accordance with the physician's plan of care As appropriate, assist the hospital social service and discharge planning personnel with the coordination of home health services Comply with regulatory compliance related to Admission/Referrals Coordinate the admission process with referral sources/sales team Obtain authorization for home health services from insurance providers, as appropriate Comply with accepted professional standards and principles Work closely with agency personnel to identify community resources Participating in Performance Improvement activities Provide support for patient/client services with home care resources as needed Maintain schedule for services requested and provided to agency patients Assure that cases are filled within established time frames Provide appropriate notification of schedule to employee, patient, contract agency, supervisor, and others as appropriate Respond to emergency calls and arranges schedule accordingly Provide support to field staff and communicate with supervisors as needed to assure safe and effective coverage of patient needs Assign agency staff to patients as directed by the care plan and service request Assure employees receive complete and accurate directions and information about patient needs Report on any scheduling changes to supervisor. Notifies patient of changes and provides supervisory support in monitoring employee performance in areas of dependability, responsiveness, timeliness, and patient concerns Provide feedback to the supervising manager regarding field staff scheduling, and performance as requested Support the verification of staff time records with schedules Perform related duties as assigned Qualifications and Requirements:
Ability to work a flexible schedule. This may include, but not limited to, after hours, weekend, holiday, and On-Call support as needed High School diploma or equivalent College graduate is preferred Two (2) to three (3) years of home health experience or other healthcare public relations, preferred Excellent communications, both written and verbal, organizational, presentation and facilitation skills Continuing Education Requirements: Agency personnel are expected to participate in appropriate continuing education as may be requested by their immediate supervisor. In addition, agency personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All agency personnel must attend mandatory educational programs. Ability to work effectively with a diverse community Complexity:
The ability to perform highly detailed work with sustained attention and care while providing/obtaining information on numerous inquiries is paramount to success in this position Frequent variety of unrelated tasks.
