Job Summary: The Licensed Practical Nurse (LPN) provides basic medical andnursing care to patients within a skilled nursing facility. The LPN works underthe supervision of a registered nurse or physician and provides direct patientcare such as medication administration, wound care, monitoring vital signs, andassisting with activities of daily living.Key Responsibilities:•Administer medicationsand treatments as prescribed by the physician or registered nurse•Monitor and recordvital signs, such as blood pressure, temperature, pulse, and respiration•Assist with theactivities of daily living, including bathing, dressing, grooming, andtoileting•Perform basic nursingprocedures, such as catheterization, tube feedings, and wound care•Document patient careactivities in the medical record accurately and in a timely manner•Communicate withphysicians and other healthcare team members to ensure continuity of care•Participate ininterdisciplinary team meetings to discuss patient care and treatment plans•Adhere to facilitypolicies and procedures, including infection control, safety, and HIPAAregulations•Maintain a clean andorganized work environment•Provide emotionalsupport and education to patients and familiesQualifications:•Graduate of anaccredited practical nursing program•Active LPN license inthe state of employment•Current CPRcertification•Minimum of 1-yearexperience in a long-term care or skilled nursing facility preferred•Excellentcommunication and interpersonal skills•Ability to workindependently and as part of a team•Strong organizationaland time-management skills•Compassionate andempathetic towards patients and families•Ability to lift andmove patients as neededPhysical Demands:The LPN must be ableto lift and move patients (up to 50 pounds), as well as stand, walk, and bendfor extended periods of time. The LPN must have visual acuity and manualdexterity to perform nursing procedures accurately and safely.Work Environment:The LPN works in ahealthcare setting that may involve exposure to infectious diseases, unpleasantodors, and verbal or physical aggression from patients or family members.
