Serve as the first point of clinical contact for walk-ins and scheduled clients requiring triage services Perform timely and comprehensive nursing assessments, including vital signs and presenting concerns Administer medications and provide medication education to clients, following established protocols Coordinate communication with prescribing providers for medication questions and refill staging Maintain accurate and up-to-date documentation of client interactions and medication records Conduct MAR reviews and manage house stock medications for applicable programs Assist in preparing clients for provider appointments, ensuring all preliminary clinical information is gathered Respond to urgent client needs with compassion and clinical skill, escalating to appropriate staff as needed Support lab duties, including collecting urine samples and performing blood draws when appropriate Participate in nursing meetings, team huddles, and quality improvement activities Perform other duties as assigned by nursing leadership Education and/or Experience Qualifications

Graduate of an accredited practical nursing program Current LPN licensure in the state of Illinois Required License/Certification (one of the following):

Valid Licensed Practical Nurse (LPN) license in good standing Current CPR certification (preferred or required by site policy) Additional Qualifications

Strong triage, communication, and documentation skills Proficient in using EHR systems and Microsoft Office Ability to work independently and within a team environment Strong attention to detail, organization, and problem-solving abilities Respect for client confidentiality and compliance with HIPAA regulations Experience in behavioral health, addiction services, or community healthcare settings preferred Supervisory Requirements
