Pathway Home Housing Navigator
The Housing Navigator for the Pathway Home Program provides direct housing support services to individuals on-site at our Pathway Home site. This role involves navigating housing systems, building relationships with landlords, and ensuring participants receive the services necessary for successful community integration. The Housing Navigator will be responsible for: Receiving referrals and coordinating intakes within an appropriate amount of time. Providing housing navigation and case management services to individuals referred through the Pathway Home program. Conducting housing assessments and development of individualized housing plans that address barriers such as credit history, criminal records, and income instability. Maintaining a caseload averaging 20 clients (can increase temporarily if programmatic needs require). Actively identifying, building, and maintaining relationships with property owners and housing partners willing to rent to program participants. Assisting clients with housing applications, including gathering documentation, submitting paperwork, and following up on application status. Assisting with move-in support, including but not limited to providing coordinating transportation and assistance with lease signing. Collaborating with property managers and other organizations to provide comprehensive services to clients. Providing warm handoffs to outside organizations and ensuring clients is properly connected to any services that will provide improvement in quality of life (substance use, mental health, medical benefits). Maintaining accurate records of services being provided and following contractual standards and HYC standards. Maintaining and executing confidential information in accordance with HIPAA standards. Participating in team meetings, supervision, training, and community outreach activities as required by leadership. Other duties as assigned by leadership. Requirements: High School Diploma One year experience working with people experiencing homelessness, preferably those living with mental illness and/or substance addictions or equivalent education and experience. Knowledge of case management techniques including trauma informed care, motivational interviewing, harm reduction, housing first model. Ability to maintain professionalism and possess a high level of tolerance and understanding for individuals with urgent and multiple case management needs. HMIS experience is highly desirable. Be willing to work flexible hours which may include evenings or weekends on occasion. Have reliable transportation, a valid driver's license, auto insurance, and the ability to qualify for Helpline Youth Counseling's insurance coverage. Clear and concise oral and written communication skills. Strong time management skills and the ability to coordinate multiple tasks with time sensitive deadlines.
