Personal Care Attendant
Jordan Valley Community Health Center (JVCHC) is a mission-driven organization dedicated to improving the health of individuals and families in underserved communities. Under the direction and supervision of the Center Manager, the Personal Care Attendant (PCA) provides home care services to Jordan Valley Senior Care participants. The Personal Care Attendant (PCA) will also participate in other activities and tasks as assigned to assist the home care, therapy, and adult day health staff. Key responsibilities include: Providing personal care and escort assistance in accordance with individual treatment plans
including peri-care, grooming, dressing, showering, bathing, toileting, feeding, ambulation. Reporting observed changes in participants' conditions to the Center Manager and/or other center-based clinical staff. Proactively communicating participant health and safety concerns to the Center Manager and/or other program staff. Demonstrating safe practices utilizing proper body mechanics and complying with universal precautions. Interacting in a friendly, courteous, and professional manner with participants and their families/caregivers. Participating in social and recreation activities with participants as appropriate. Implementing interventions, as appropriate to training, as directed by rehabilitation staff, including use of mechanical assistance for participant transfers and mobility. Occasionally accompanying participants to outside clinical appointments, when necessary. Assisting with food service delivery and monitoring during mealtimes.
