The Behavioral Health Specialist is primarily responsible for providing case management for the Behavioral Health patient population. The Specialist follows the BH program's Scope of Work to achieve the program goals through the screening of patients, referral organization, referral management, appropriate linkage of care, and record keeping. The Specialist manages cases within the Behavioral Health patient demographic. The Specialist adheres to the BH program's Scope of Work to accomplish its objectives by screening patients, implementing interventions, conducting more personalized one-on-one sessions, organizing and managing referrals, ensuring proper care linkage, and maintaining records. Minimum Requirements
A Bachelor's Degree in Social Services, Psychology, Sociology, Human Services, or Counseling is highly preferred. Minimum of 3 years working in health care supporting patients with case or referral management and linkage to care. Bilingual English/Spanish or English and other predominant language of the population served preferred. Compensation
$30.82 - $38.53 hourly Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations. The range is subject to change.
