Program Statement: Our Outpatient Behavioral Health Department provides diagnostic assessment, psychiatry, counseling and case management services to individuals experiencing challenges related to mental health, substance use disorders, and barriers such as homelessness, lack of community benefits, and lack of support. We provide case management services with a focus on supporting those with special needs, including individuals with disabilities (developmental, mental health, chemical dependency, HIV/AIDS, and the elderly) and co-occurring disorders. Our staff helps to identify and address the unique needs of our clients: from finding employment, or enrolling in benefits, addressing their medical and/or mental health concerns, and accessing eligible housing. The ultimate goal of our program is to help each individual secure stability and improve self-sufficiency in the community or the least restrictive environment of their choosing. Responsibilities: Provide coordination and linkage to area community resources, assisting individuals to re-establish appropriate identification such as birth certificates and state ids/driver's licenses. Provide housing assistance by working with LMHA, HUD, and community landlords to complete application requirements. Provide advocacy and support to individuals with mental health and addiction issues. Assist in helping to apply for employment and/or apply for financial benefits through Job and Family Services. Link and coordinate medical appointments in the community based on client need. Monitor clients who have become housed in the community for continued housing retention. Requirements: Combination of education and experience normally represented by a Bachelor's Degree and one to two years related professional experience. Must have competent oral, written and interpersonal communication skills. Must be well organized, detail oriented and have the ability to maintain confidentiality. Ability to work with diverse client population, community organizations as well as staff at all levels. Must be able to form good working and therapeutic relationships.
