The role of the Medical Social Worker is to work with the interdisciplinary team to ensure a safe discharge of all adult patients. Duties include but are not limited to: Identifying patients with barriers to a safe discharge plan. Completing psychosocial assessments on all patients. Documenting all clinical assessments, notes, and referrals in Epic. Completing screens and coordinating all new skilled nursing facility and rehab facility referrals. Assessing all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault. Educating patient/family about post-acute care options and the role of choice. Educating patient/family about managed care and payor issues This is a full-time day shift position 9:00am-5:00pm Monday-Friday including every 4th weekend, both Saturday and Sunday, located at NewYork-Presbyterian Brooklyn Methodist Hospital. Preferred Criteria: At least one year of experience in an acute care setting. Discharge planning experience. Experience with Epic and Allscripts. Requirements: Master's Degree in Social Work (MSW). New York State License as a Licensed Masters Social Worker (LMSW). Communication and negotiation skills. Ability to work independently and collaboratively with the interdisciplinary team. Work efficiently and effectively under pressure. Work comfortably within the limitations of the medical setting. Computer literacy.
