Case Manager/Social Worker
Coordinates the delivery of rehabilitation services to patients in collaboration with the facility staff. Continually monitors and documents the cost effectiveness of treatments provided, facilitating admission and discharge processes, serving as patient advocate and liaison to insurance carriers and other medical management professionals. Qualifications: Current California LVN or RN license in good standing Minimum 3 years experience in a hospital, nursing, or other related health care facility Working knowledge of managed care rules, regulations, and guidelines Outstanding communication (oral and written) skills Ability to multi-task and treat everyone at work kindly and professionally Works collaboratively and independently, following procedures and guidelines Completes tasks on time, reporting on successes, challenges and offering solutions Emotional maturity
ability to remain calm and professional under stress Responsibilities: Greet new patients/residents ensuing a positive admission experience Evaluate and communicate resident status, changes in functions and care plans to payers Document all payer interactions regarding resident progress, expected outcomes and special instructions Meet with interdisciplinary team to coordinate services ensuring resident's total regimen of care is maintained Coordinate and communicate with business office and medical records to ensure accurate census and payment of managed care and Medicare residents Benefits: Competitive pay Health Benefits
