Kidney Transplant/Chronic Kidney Disease Clinics. Call required - shared between Transplant Coordinator team. The Transplant Coordinator is responsible for the coordination of general program and patient care activities. The Transplant Coordinator establishes and promotes liaison relations with referring facilities and provides program information, patient follow up and education programs. The Transplant Coordinator serves as a consultant to AC staff as well as follow-up service providers and as a liaison to governmental and other regulatory agencies. The Transplant Coordinator maintains the necessary data bases relative to organ recipients, tissue typing, patient status, etc. Additional Information:
Required Education:
Associate's degree or equivalent from two-year college program or technical school or nursing diploma, Associate's Degree or Equivalent or Nursing Diploma, Associate Degree Nursing Recommended Education:
Bachelor's Degree, Bachelor's Degree Nursing, Bachelor's Degree or Equivalent or Nursing Diploma Required Work Experience:
Nursing - 4 years of experience Recommended Work Experience:

Blood and Marrow Transplantation Certified Nurse (BMTCN) - Oncology Nursing Certification Corporation, Certified Clinical Transplant Coordinator (CCTC) - American Board of Transplant Certification (ABTC) Description:
1. Serve as a liaison between patients and their family members, transplant team, nursing and laboratory personnel and other support personnel. 2. Coordinates activities associated with evaluation for transplantation. 3. On call for solid organ or donor cell availability. Responds to transplant calls within 20 minutes. 4. Coordinates procurement and care of the transplant patient pre and post transplant. 5. Assist with long-term post transplant patient assessment and management in the clinics. 6. Makes rounds on transplant service patients to observe patient progress, identify actual or potential problems and initiate problem solving actions. 7. Performs or assists with patient procedures including venipuncture for laboratory studies or intravenous infusions, care and dressing of wounds. 8. Evaluates psychological response to both illness and medical/nursing interventions. Provides support, comfort and encouragement to patient/family/significant other as indicated. 9. Responsible for pre-transplant testing and collaboration with applicable organ/cell matching organizations. Must be able to participate in pertinent on-site inspections/audits. 10. Responsible for ongoing input and analysis of benchmarking data into appropriate national databases. 11. Must be able to coordinate and evaluate living donor workup/follow-ups. 12. Collaborates with team to provide patient/family education pre and post transplantation. Collaborates in the planning, organizing and implementation of the orientation program for all personnel on the nursing units and team. 13. Conducts nursing research and collaborates in medical research programs. 14. Identifies and actively participates in quality improvement initiatives for the transplant program. 15. Maintains data bases relevant to specialty areas and areas of investigation in collaboration with primary investigator. 16. Performs other duties as assigned.
