Description
The
Housing Case Manager
provides homelessness support and harm reduction services to individuals at risk for infectious disease, overdose, and other health conditions associated with substance use, human trafficking and/or unstable housing. Services include housing case management, HIV/HCV and STI testing, PrEP education and linkage services, syringe exchange, naloxone distribution and overdose education.
The
Housing Case Manager
participates in program-based outreach, community meetings and facilitates psychoeducational groups. This position reports to the Program Director for support and supervision.
Location: The Victory Connector
is a harm reduction, navigation, and respite center for women and transgender individuals who use drugs and/or are experiencing homelessness and at risk of exploitation. The Victory Connector Team works to improve outcomes, reduce mortality and enhance overall quality of life for individuals experiencing homelessness and exploitation, who are impacted by substance use, behavioral health issues and infectious diseases by seamlessly connecting them to resources and knowledge through navigation, outreach, and education.
Essential Functions
Act in accordance with Victory Programs, Inc.’s Standards of Excellence, including maintaining professional ethics, boundaries and client confidentiality. Ensure that the policies and procedures of the programs are followed. Identify housing support needs; coordinate and advocate for appropriate support services and resources that will assist clients in optimizing health outcomes and housing stabilization, moving towards self-sufficiency. Provide program support, including but not limited to harm reduction supply distribution, milieu engagement, group facilitation. Provide housing stabilization services to clients including eviction prevention, recertifications, accessing funding and benefit sources. Accompany outreach staff on street and venue-based outreach to provide low barrier housing services and navigation. Accompany clients to resource-related appointments, appeals, and following up on submitted client applications. Engage in relationship building, Nonviolent Crisis Intervention, Motivational Interviewing and conflict de-escalation as needed in the milieu space. Act as a liaison between clients, property management companies, medical personnel, volunteers and families; navigate and identify different components of a client’s support system at large. Develop and coordinate a system for follow-up/outcome measurement to track housed clients. Maintain timely and accurate records of all client work, completing all the program required assessments. Maintain clear, concise and accurate client files and data-based records. Accompany clients, as needed, to housing-related appointments. Facilitate a monthly aftercare group to assist in the maintenance of housing stability. Maintain contact with other internal and external client providers, including, but not limited to counselors, case managers, and housing providers. Meet clients where they are permanently housed or in the community as needed, providing weekly case management unless otherwise noted. Continue to assess and provide referrals and other community-based recovery, health, nutritional and other support services for individuals receiving stabilization services. Develop and maintain service linkages to support clients/residents and their families, including developing relationships with local service providers. Remain in good standing with agency and funder required trainings. Promote access to and participation in the community and neighborhood, including gathering and distributing information regarding community meetings, events, and resources. Attend weekly supervision, monthly staff meetings, and periodic professional development training and workshops as requested by the supervisor. Participate and support the agency in program events, State House rallies, fundraising and advocacy events, rallies, walks and any additional functions. Attend additional training as requested by your supervisor to enhance skills. Perform any other duty or task that may be requested by a supervisor or the Program Director. Represent Victory Programs, Inc. in external meetings, as requested.
Essential Qualifications
Bachelor’s degree with 2 years’ experience, or High school diploma with 3 or more years of housing case management experience. Working knowledge of HIV/AIDS and viral hepatitis, homelessness, substance use, STIs, and sex work, mental health, as well as health-related social issues faced by persons who use drugs. Ability to work effectively with individuals of diverse race/ethnicity, sexual orientation, gender identity, sex and drug cultures, age groups, religion, country of origin, disability and economic status. Excellent verbal communication skills including an ability to have open and affirming conversations about sex and substance use risk. Previous experience in nonviolent crisis intervention, de-escalation, and milieu management strongly preferred. Previous experience working in human services and/or a drop-in setting. Experience advocating for clients with government assistance agencies, housing authorities, private landlords or realtors. Demonstrated competence in service coordination and networking with other human service and health care providers. Ability to work independently and cooperatively as a member of a professional team. Strong computer skills, including Google office suites and Internet use. Must be CPR certified (may be obtained upon hire) Complete TB screening (may be obtained upon hire) Three strong work-related professional references; at least 2 from manager or supervisor.
Preferred Qualifications
Bilingual (English/Spanish, Haitian Creole) preferred. A valid Massachusetts driver’s license with a safety driving point of 00, 99, 01, 02, or 03.

These figures may vary based on full-time or part-time status, and vacation time increases with length of employment.
