Position Summary
The Program Manager is responsible for operational activities of HRSA funded, RYAN WHITE PART D program. The goals of the RYAN WHITE Part D include monitoring the transition process for adolescents/young adults to engage in care in the adult HIV program; enhancement of the transition of pregnant women living with HIV/AIDS to the adults’ program after delivery; retention of adolescents, young adults, and newly identified women in care; and monitoring quality of care for HIV infected children, youth and women. The Program Manager provides training, guidance and oversight to assigned staff.
Responsibilities
The Program Manager will also perform medical case management and be responsible for carrying out the goals of the HRSA Ryan Project with regards to Women Living with HIV/AIDS. Major targets include:
Engagement of pregnant women living with HIV/AIDS into care, and addressing barriers to care such as health insurance enrollment and transportation.Engagement of targeted women living with HIV/AIDS at risk of falling out of care or at risk of incomplete immune reconstitution (CD4
