Nurse Practitioner (PRN) (Cook County)

Avante Group Inc. Seeking a Nurse Practitioner to perform annual risk assessments in homes or via telehealth, within their scope of practice.
Conduct comprehensive health assessments, identify active and chronic conditions, and evaluate physical, mental, and social needs. Take patient histories, examine, diagnose, and document visits per standards. Record vital signs, coordinate case management, and communicate with healthcare team and caregivers. Maintain patient confidentiality, adhere to OSHA safety standards, and participate in professional development. Work with teammates, physicians, patients, caregivers, nurses, and vendors.
Qualifications include an active nurse practitioner license, board certification, CPR certification, good driving record, and strong EHR skills.
Preferred experience in geriatrics and teamwork skills are a plus.
Role involves adaptable home environment visits, physical mobility, and driving up to 150 miles daily. Seniority level
