Position Purpose: The Clinical Manager assists the home health director to maintain operational aspects of patient care for all hospice patients. Under administrative direction, this position assesses plans and evaluates delivery of patient care and directs and evaluates nursing functions. The Clinical Manager evaluates action plans and provides input to director to help ensure that nursing practice continually improves and has a positive impact on patient outcomes. This position is FLSA exempt.

- Serves as a resource to staff and ensures that patient care provided is according to established policy and procedure.
- Ensures scheduling is completed and all patients have specific case managers and designated team members.
- Ensures Interdisciplinary Team (IDT) meetings are held weekly and documented.
- Demonstrates the ability to support staff with problem solving and decision making.
- Ensures that equipment and supplies efficiently managed and available for patient care.
- Promotes open communication within the department and with other KRMC units and departments.
- Has knowledge of patients currently on hospice and their clinical conditions. Communicate as needed with family members regarding care provided to patients.
- Assists with Performance Improvement activities, auditing documentation and processing of computer documentation. Conducts annual competencies for all staff in hospice.
- Works to meet Patient, Physician and Staff satisfaction goals by setting example, and contributing to teamwork with all hospice staff.
- Investigates incidents which may occur and ensures that appropriate documentation and corrective action is taken.
- Performs regular audits of nursing documentation. Evaluates nursing care through assessment, planning, intervention, and reevaluation.
- Treats physicians with courtesy and respect. Works to meet physician expectations.
- Serves as clinical resource person. Assists in monitoring staff attendance at mandatory in-services, IDT meetings and staff meetings. Assists director by recommending in-service needs of staff.
- Serves on committees as requested by director in KRMC such as VAC committee and Readmission prevention rounds. Shares knowledge gained with department staff.
- Participates in Quality Assurance and Process Improvement (QAPI) programs.
- Seeks knowledge and experiences to expand and maintain skills and knowledge base, clinically and specifically for hospice. Participates in educational programs and activities, conferences, workshops and self-directed learning.
- Regularly engages in self-performance appraisal, identifying areas of strength as well as areas for professional development. Seeks constructive feedback regarding his/her, own practice.
- Participates in orientation of new staff by meeting appropriately and timely with new staff and preceptor to provide input and evaluate progress.
- Assists director by providing input regarding employee evaluations and applying corrective action as directed.
- Works with director to ensure patient/family satisfaction and is responsive to patient/family requests.
- Complies with all federal, state, organizational, and departmental policies, procedures, and requirements.
- Education: Graduate of a nursing degree or nursing diploma program
- Licensure:
- Arizona RN License or compact state RN license
- Level One Fingerprint Clearance Card, required within 20 days of hire
- AZ Driver's License
- AZ Food Handlers Card
- Certification: BLS through Arizona Heart Association or American Red Cross
- Age Requirement: Must be 21 yoa or older
- Experience: Minimum of three (3) yrs RN hospice or home care experience with at least one (1) yrs of supervisory experience
Work Requirements: Able to handle multiple priorities simultaneously, possess good oral and written communication skills, care for patients in a hospice setting. Able to sit 3-5 hrs, stand 3-5 hrs, and walk 3-5 hrs per day. Expected duties have routine potential for exposure to blood, body fluids or tissues; Requires travel, in properly insured vehicle, throughout our service area. Requires interaction with a variety of customers in a variety of settings.
