Licensed Practical Nurse (LPN) – Adult & Geriatric Care Overview: The LPN is responsible for delivering skilled nursing care to adult and geriatric residents in accordance with established policies and under the supervision of a Registered Nurse. This role requires the ability to assess, plan, implement, and evaluate care based on the developmental and age-specific needs of the patients served.

Key Responsibilities:

Provide direct and indirect nursing care for assigned residents, promoting a safe and supportive environment.

Demonstrate a comprehensive understanding of growth and development across the life span, especially in adult and geriatric populations.

Collect and communicate data related to residents’ physical and psychosocial condition to the RN and interdisciplinary team.

Participate in the creation and revision of individualized care plans, ensuring they are age-appropriate and culturally sensitive.

Collaborate effectively with residents, families, and healthcare teams to support optimal outcomes and satisfaction.

Maintain clinical competency through ongoing education and in-service programs.

Provide leadership and mentorship within the scope of practice under RN direction.

Consider safety, cost, and patient impact in all aspects of care delivery.

Comply with infection control protocols and understand the potential for occupational exposure to bloodborne pathogens.

Qualifications:

Licensed Practical Nurse (LPN) with active licensure

Skilled in geriatric nursing care

Strong interpersonal and communication skills

Commitment to continuing education and clinical excellence
