Job Description Job Title: Care Manager Profession: Licensed Master Social Worker Specialty: - Duration: 9 months Shift: Telephonic and/or face-to-face care management services Hours per Shift: 35.00 Experience: Minimum of 1 year LMSW experience License: Valid Licensed Master of Social Work (LMSW) license issued by NYSED Certifications: Primary Source Verification required
Must-Have: - Licensed Master of Social Work (LMSW)
Description: Provides telephonic and/or face-to-face care management services to members. Conducts care coordination, transition of care, outpatient follow-up, and ancillary service reviews. Develops and implements an individualized member Plan of Care utilizing various resources. Focuses on health coaching and care coordination activities aligned with member needs. Promotes access to and oversees the coordination and delivery of comprehensive healthcare services. Provides health education and coaching on chronic co-morbid illnesses. Communicates with members' primary care providers and applicable community case managers. Identifies service utilization trends and potential member needs through various data reviews. Documents goals, interventions, and care coordination activities in the electronic health record. Mentors and supports the work of Health & Wellness Advisors. Participates in the competency development of Health & Wellness Advisors. Attends and prepares for departmental case conferences as assigned. Reviews and assesses member Plans of Care and recommends changes for health promotion. Assists with resolution of member concerns related to quality management and customer service. Attends approved in-service and external education and training as directed. Performs additional special projects, including audit review, quality improvement, and community health education. Ensures that coordination of care activities for the assigned caseload complies with relevant regulations.
