In This Dynamic Role, You Will

Assess patients' health, diagnose conditions, and collaborate with physicians on comprehensive treatment plans, Manage chronic conditions, treat acute illnesses, and respond to urgent medical needs, Conduct health screenings, administer immunizations, and perform routine physical exams, Accurately document patient information and communicate effectively with patients and the healthcare team, and Recommend innovative tools and devices to improve patients' quality of life.

To Be Successful In This Position, You Must Possess

Board Certification as an Adult or Family Nurse Practitioner, Active NY state license, Minimum of 1 year of relevant experience including Nursing Homes, Excellent communication and interpersonal skills, and Strong organizational abilities and effective time management.
