Travel between multiple Long Term Care Facilities in Guilford County, NC

Schedule : Monday-Friday (8am-5pm)

Contract : 3-4 month contract

Pay Rate : $66-70/hour

Specific Duties:

Provide specialized care to high-risk health plan members Work as part of an interdisciplinary team Conduct comprehensive assessments Manage acute and chronic medical and behavioral conditions Ensure accurate documentation, including ICD-10 coding Communicate and collaborate with the care team Facilitate advanced illness and care planning discussions Offer counseling and education to patients and caregivers

Must have qualifications/experience:

Nurse Practitioner or Physician Assistant NP Certified in one of the following (FNP, ANP, GNP) Access to transportation 1+ year of experience (LTC, SNF, Geriatric preferred)
