Job Description
SUMMARY OF DUTIES AND RESPONSIBILITIES:
Performs medical services within the scope of approved privileges. Document all patient contact in the EMR in a timely manner. Assumes responsibility for patient care. Collaborates to provide care coordination activities in providing comprehensive care (e.g., other healthcare providers, local health departments, social service, nutritionist, dentist, community-based organizations, home care and patient education groups). Provide relevant health education and instructions, counsels patients/family responsible on the impact of illness and principles of health promotion and maintenance. Provides clinical and didactic instruction to professional staff, peers, students and community groups and conducts in-service educational programs, as needed. Participates in departmental meetings and interdisciplinary community committees, as well as peer review performance improvement meeting activities. Follows Gotham, NYC H+H and facility specific policies and protocols. Maintains expected standards of patient information confidentiality as per HIP AA policy Ensures credentials are maintained. Maintain patient safety and infection control protocol as per policies. Required to complete annual in-services. Able to work evenings and weekends. Refer to specialties. Care gaps/Care gap metrics. Completion of assigned work ques in EPIC. Completion of patient forms. Perform administrative, professional and consulting duties as requested by the Medical Director.
Minimum Qualifications
A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and Current certification to practice as a Nurse Practitioner issued by the NYSED; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
