Provides quality speech language pathology services to patients and families in the home in accordance with state licensure, laws, professional standards and ethics within guidelines of agency standards, policies and procedures, and coordinates care with all disciplines involved. As a Speech Language Pathologist in Homecare, you will play a vital role in supporting our clinical team, ensuring our patients receive the best care by: Demonstrating competent theoretical and technical skills pertinent to patient populations being served, including Dysphagia, Speech/Language Pathology, Communication skills, Cognition, Voice, and Feeding skills. Providing timely and thorough documentation of the patient record consistent with agency standards. Practicing within the Scope of Practice and complying with all policies, laws, licensure, regulations, and standards for care pertaining to the delivery of home care services. Demonstrating compliance with patient care coordination, including weekly and monthly chart audits, utilization, infection control, improvement in corrective action plans, and attendance at staff and training meetings. Demonstrating effective primary patient care coordination with all disciplines when assigned. Thoroughly identifying and evaluating functional and rehabilitative needs. Identifying and prioritizing patient needs. Developing and implementing appropriate care plans and goals. Adjusting and updating care plans as needed. Appropriately coordinating patient care with other disciplines. Providing appropriate education to patients and families. Consistently following the established care plan. Demonstrating the efficient use of resources by effectively managing time, supplies, and equipment and by meeting established productivity standards. Minimum Qualifications Valid Nevada Speech Language Pathology License. Current driver's license, insured, reliable transportation, and acceptable driving record. Basic Life Support (BLS) certification for healthcare providers. Ability to access the Internet or a phone line for transferring patient files twice a day. Effective communication skills. Basic computer skills. Current certificate of clinical competence. Preferred Qualifications One (1) year of experience. Physical Requirements: Hearing/Listening, Manual Dexterity, Pulling/Pushing, Seeing, Speaking.
