As a key member of our team, you'll assess patients, develop care plans, and monitor progress to ensure optimal outcomes. Your responsibilities also include: Managing a caseload of home health patients, ensuring effective assessment and planning. Prioritizing patient needs and adapting care approaches for dynamic situations. Conducting comprehensive assessments of patients' physical, functional, and psychosocial status. Developing individualized care plans in partnership with patients, families, and physicians. Educating patients and their families on managing care safely and effectively at home. What We Offer: This is a full-time position with a schedule of Monday-Friday from 8am to 4:30pm, including on-call weekend commitments monthly or quarterly. Location:

Jackson, Platte, Clay, Cass Counties in Missouri and Leavenworth, Johnson, Wyandotte, Franklin, Douglas, and Miami Counties in Kansas.
