Pair Team Pair Team is seeking a highly motivated full-time Registered Nurse Care Manager to support patient-driven care plans and improve outcomes for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and/or high medical needs. The RN Care Manager will work in a team-based model alongside a lead care manager, community health worker, behavioral health care manager, and nurse practitioner. Responsibilities include managing a caseload of individuals with complex medical needs, identifying health and wellness goals, educating patients on medical and behavioral health conditions, medication reconciliation, coordinating prescriptions and prior authorizations, tracking assessments and screenings, collaborating with multidisciplinary care teams, triaging clinical needs, participating in case reviews, providing educational training, and building trust with underserved populations. The role requires empathy, cultural fluency, and the ability to engage individuals who may be hesitant or distrustful of the healthcare system. The position is primarily remote with 1-2 monthly on-site community visits. Candidates must hold an active California RN license, be bilingual in English and Spanish, have 2-3+ years of relevant experience, and possess strong technical skills and a HIPAA-compliant home work environment.
