opportunity is based out of a prominent medical center and rehabilitation campus offering exceptional short-term rehabilitative services in a supportive, team-based environment.
POSITION OVERVIEW:
The Case Manager plays a key role in managing short-term rehabilitation stays by coordinating care authorizations and facilitating interdisciplinary collaboration. This individual ensures smooth communication between healthcare teams and insurance providers to maintain uninterrupted patient care. RESPONSIBILITIES: Coordinate authorization and continued stay reviews for patients covered by HMO or commercial insurance Advocate for extended care by interpreting medical records and clearly communicating patient needs Partner with interdisciplinary teams to gather clinical updates and meet time-sensitive insurance deadlines Maintain proactive relationships with payer case managers to secure timely approvals Anticipate coverage trends and align discharge planning with insurance parameters Ensure documentation accuracy, deadline compliance, and seamless internal communication
QUALIFICATIONS: Experience in a rehabilitation, skilled nursing, or acute care setting Familiarity with insurance authorization processes, especially for HMO or commercial plans Strong ability to interpret medical documentation and advocate for continued services Effective multitasker who can manage timelines, deadlines, and competing priorities Strong interpersonal and communication skills for both internal collaboration and external advocacy
COMPENSATION AND BENEFITS:
This position offers a competitive annual salary range of $75,000 to $90,000, depending on experience.
