Position Summary instED provides on-demand, urgent care in the home for medically complex patients. The RN, instED Clinical Resource Center will receive referrals for services, conduct intake assessments, provide triage to ensure referrals are clinically appropriate, and respond to clinical questions related to referrals and services. The InstED RN will engage telephonically with patients, clinicians, and care partners who call with urgent clinical needs for a medically diverse group of patients across the continuum of care.
Responsibilities
Intake:
Receive referrals, ensure complete and accurate information, obtain data, triage per guidelines, and provide competent telephonic nursing care.
Triage:
Make appropriate recommendations, prioritize referrals, identify emergent situations, analyze data, develop plans, and exercise sound clinical judgment.
Follow-up:
Communicate with patients, provide education, ensure teaching, document accurately, and focus on patient experience.
Secondary Responsibilities:
Participate in quality assurance and program support activities.
Working Conditions Remote environment with flexible schedule; shifts include weekends and holidays, coverage 365 days/year.
Qualifications
Bachelor’s degree in Nursing
Current, active MA RN license
5+ years of relevant nursing experience
Strong triage skills, excellent communication, and ability to work collaboratively
Willingness to learn telehealth technologies
English language proficiency; Spanish is a plus
Additional Details Position level: Mid-Senior; Full-time; Healthcare Provider industry; Boston, MA location.
