Minimum Qualifications for a lease agreement: Doctor of Optometry (OD) degree from an accredited school
Valid state optometry license
Ophthalmologist (MD or DO) from an accredited school
Valid state ophthalmology license
General Liability Insurance Certificate
Professional Liability Insurance Certificate
