Description
Supervise Residential Individuals' ISPs: Ensure that all staff have read, understand, and implement the Individual's Individual Support Plan. Encourage individual independence. Maintain individual/program confidentiality. Ensure that all data on the progress of ISP Goal are recorded accurately and in a timely manner. Monitor progress and make suggestions for changes or amendments needed to assure individuals' ISP success. Maintain good communications with individual's families and other ISP team members as well as the home staff.
Supervise Residential Individual: Medical Issues. Monitor to see that all individuals' medical appointments are kept up to date per Dr. recommendations. Ensure that all pertinent information such as lab results, consult sheets, and any other test results are obtained in a timely manner, copies given to DPM and Support Coordinator,and kept in the individual's home file. Order all individuals' medications, reorder of routine as well as all new medications prescribed and monitor for accuracy. Check all M.A.R.'s at least weekly for accuracy and report issues to DPM. Communicate with our RN Consultant, DPM, and Support Coordinator regarding concerns about the individual's health and follow their suggestions.
Supervise Residential Individual Activities of Daily Living/Social Interactions . Monitor individual's activities to insure community involvement is occurring. Monitor to ensure that staff is encouraging individuals to participate in or complete as many personal chores as they are able physically able to participate in, ex, bathing, hair care, oral care and as many household chores they have the ability to accomplish. Monitor to ensure that staff is respectful of each of the individual's, their choices, wishes, suggestions and decisions about daily activities. Monitor and see that all individuals are in contact with those persons who are important in their lives.
Supervise Staff Personnel Scheduling Issues. Participate in and offer input in the interview of potential new hires as requested by supervisors. Provide one-on-one training for all new staff and retraining for all staff as needed. Develop bi-weekly staff schedules keeping in mind individual needs over staff convenience. Post staff schedules in ADP by the end of the day Wednesdays following payroll week. Give a copy of your schedule to DPM by the same time. Inform DPM of changes to schedules, in a timely manner, as needed after schedules are complete. Turn in time and schedule adjustments as they occur to DPM, ensure that staff turnin time and schedule adjustments accordingly. Monitor for accuracy and approve all timesheets, expense vouchers and vacation/PTO requests.
Supervise Staff Personnel Growth/Conflict/Discipline. Encourage staff growth by delegating duties and monitoring for completion, offering supports as needed to assure successful outcomes. Monitor all staff related issues/problems and report to the supervisor. Follow the Employee Counseling and Disciplinary Guidelines per agency policy. Complete Performance Reviews on all Residential Instructors and submit to DPM and Executive Director for review. Complete department staff meetings at least quarterly to discuss individual/ department concerns, progress, up-dates, items needed, informational reminders, trainings, etc. Regularly keep good communication with staff regarding changes and up-dates as they occur. At times it may be necessary to keep documentation with signatures that the information was shared.
Supervising the Home: Finances. Monitor and balance all individual accounts, grocery/supplies and personal monies weeklyand bring reports to Finance Manager monthly. Monitor all schedules and work out any overtime issues for preventing un-necessary wasted budget dollars. Communicate any overtime or potential overtime to DPM and/ or Executive Director. Maintain and monitor all required certifications for you and your staff and report all needed re-certifications to Human Resources Manager. Monitor home for cost cutting measures such as thermostat control, electric/water conservation and bargain shopping.
Supervising the Home: Documentation. Monitor all documentation such as individual case notes, flow charts, daily activities, safety checklists and weekly menus for accuracy and completion. Assure that all staff expectations is clearly outlined, such as daily, weekly and monthly cleaning lists and monitor for completion. Complete monthly reviews on all individuals and submit to Support Coordinator and DPM by the 15th of each month, keep a copy for the home file. Maintain good communication with home staff even when not on duty. Assure that all staff is aware of whom to contact during your absence/off time/vacations, etc.
Supervising the Home: Safety. Ensure that staff completes monthly emergency drills and check lists and submit copies to and Safety Director or Co-Safety Director, in a timely manner. Monitor home and surroundings for cleanliness, and all appliances for repair and condition. Monitor home for clutter buildup; see that all carpets/floors are clean, and all hallways and walkways are free from trip/fall hazards. Monitor all Safety and Adaptive Equipment, ex, fire extinguishers, smoke and fire alarms, individual lifts/wheelchairs/ramps are functional and in good repair Monitor all temperatures, ex, Water, Refrigerator and Freezer and food storage areas to assure proper storage for food and safety for the individuals. Complete Work Orders for any repairs needed made, turn in to Maintenance Director as they occur.
Food Safety: Monitor pantry shelves for expiration dates on foods and assure that supplies are rotated so that all foods are used in a timely fashion, at least monthly. This includes all Frozen Foods. Monitor to assure that all leftover food is labeled with proper expiration dates, and that they are used within acceptable time-lines, or that they are discarded properly. Monitor all staff to assure they are maintaining safe food preparation, ex, using good hand washing, cooking food to proper temperatures and storing food appropriately.
Job Requirements : Availability. Be able to work mornings, evenings, overnights, weekends and holidays. Understanding that the Home Manager position requires you to be On Call for your individual, staff and supervisors 24/7. (Exception would be during vacations). Be able to work with all individuals and be able to handle bodily fluids, ex, urine, vomit, drool, etc. Be able to bathe, and or change adult diapers if needed. Attend mandatory trainings, meetings and in-services as requested to acquire and maintain certifications. Some of these trainings/meetings could be out of town and require you to stay overnight. Have and maintain a legally acceptable and licensed vehicle and be willing to transport individuals in said vehicle. Be able to acquire a Class "E" Chauffeur's License within 10 days of employment. Have and maintain a working telephone where we can reach you in case of emergencies. Follow all company Policies and Procedures as written in the Manual including proper lifting and moving procedures utilizing company supplied Safety Equipment and or Materials. Accurately complete and submit employee timesheet and expense vouchers as required. Perform all other related work duties as assigned.
