RecruitWell is seeking a compassionate and experienced Nurse Practitioner (NP) to join a mission-driven team delivering holistic, value-based care to Medicare patients in the Medford, OR area. This permanent, full-time role focuses on annual wellness assessments in both outpatient clinic and in-home settings.
Position HighlightsConduct annual Medicare wellness visits in clinic or at patient homes (within 30-mile radius)
Manage high-risk and post-acute patients to reduce avoidable hospitalizations
Perform HCC coding , screenings, discharge planning, and medication reconciliation
Collaborate with an interdisciplinary team (social workers, care managers, MD advisors)
Average 10-12 clinic or 5-6 home visits per day
Complete EMR documentation within 72 hours
Active Oregon NP license
Certified by ANCC or AANP
2+ years NP experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or Palliative Care
DEA license , CPR/BLS certification, and valid Oregon driver's license
Experience with HCC coding , managed care, or home health is a plus
Passionate about geriatric and underserved populations
Tech-savvy, detail-oriented , and thrives in a structured environment
Open to feedback and committed to continuous improvement
Enjoys building meaningful patient relationships and delivering empathetic care
