AbsoluteCare is seeking a dynamic, mission-driven clinical leader to implement team-based collaborative care and population health principles for our medically complex and socially vulnerable members. We provide innovative, value-based care to traditionally underserved patients, aiming to improve quality of life and health outcomes while reducing unnecessary costs. Our integrated team includes primary care providers, care managers, nurses, behavioral health clinicians, and community health workers, with onsite infusion, pharmacy, labs, and imaging services. We also address social determinants of health such as housing, food security, and social isolation.

The Lead Physician provides direct clinical care, guides best practices, and collaborates with the Medical Director for clinical leadership across providers, nurses, behavioral health specialists, and medical assistants. Key responsibilities include:

1. Providing primary care to a panel of members and leading interdisciplinary care teams.
2. Coaching and mentoring providers in delivering excellent primary and urgent care, and leading population health initiatives.
3. Offering clinical guidance to staff on complex cases and participating in leadership meetings.
4. Representing the center in community engagement and partnerships with hospitals, organizations, and payers.

Additional duties encompass:

• Delivering comprehensive primary care services.
• Collaborating with multidisciplinary teams to develop and implement care plans.
• Mentoring physicians and advanced practice providers.
• Managing acute exacerbations and ensuring appropriate care levels.
• Tracking patient outcomes to prevent hospitalizations and readmissions.
• Practicing evidence-based medicine aligned with the PCMH model.
• Achieving quality metric goals and utilizing electronic health records effectively.
• Participating in on-call rotations and developing clinical protocols.
