Summit County Public Health is seeking a Project Specialist to work full-time (35 hours per week) , Monday through Friday, in our Senior & Adult Services program.

This position provides direct services to participants in the the BRI Care Consultation program, which offers coaching and support to individuals with dementia and their caregivers. It also supports the Gatekeeper Program, which helps identify and assist older adults with suspected or known dementia or Alzheimer's disease who are living alone or have intellectual developmental disabilities-promoting their safety and independence.The role involves case management, assessment, and resource coordination. In addition to direct service, the position provides community education on brain health, fall prevention, dementia awareness, and caregiver support. Experience working with older adults in a supportive or caregiving role is highly valued. Provide direct services to program participants, including intake, case management, and ongoing support to help older adults live safely and independently. Support the planning, implementation, and evaluation of programs and initiatives within Senior & Adult Services. Coordinate and deliver community education on brain health, dementia awareness, caregiver support, and fall prevention. Research, recommend, and apply best practices in public health and aging to improve policies, systems, and environments. Administer and monitor grant components, including applications, strategy execution, documentation, and compliance. Collaborate with community partners, participate in coalitions, and support community-based organizational change efforts. Identify program barriers, assess community needs, and recommend solutions or funding opportunities to improve services. Assist in tracking and reporting program growth, outcomes, and alignment with grant or program objectives. Communicate effectively in writing and verbally across diverse populations and settings. Demonstrate knowledge of public health principles and independently manage tasks with sound judgment and integrity. Utilize Microsoft Office and Google tools for program administration, documentation, and communication. Participate in training, emergency preparedness activities, and perform other duties as required to support agency operations. Qualifications:

Bachelor's degree in nursing, dietetics, social work, public health, or related field. Must possess a minimum of 1-2 years related work experience. Valid Ohio driver's license, automobile availability and insurance coverage.
