Organization: SEBMF-East Bay Medical Foundation Position Overview: Provides medical nutritional therapy (MNT) and Diabetes Self-Management Education (DSME) to patients with diabetes, pre-diabetes, and related conditions such as cholesterol, renal, and endocrine disorders. Leads the daily activities of Clinical Health Educators, collaborates with treating physicians to manage treatment and monitor responses, and provides education and consultation to nursing and ambulatory staff. Supports physician and clinician education in hospital and patient care settings. Offers nutritional care (MNT) and counseling to adults and children through clinician referrals and works with the healthcare team. Job Description Education: Bachelor's degree in Nutrition, Dietetics, or a related field Or graduate of an accredited nursing school Certification & Licensure: RN-Registered Nurse (California) or RD-Registered Dietitian CDCES-Certified Diabetes Care and Education Specialist BLS-Basic Life Support Healthcare Provider Typical Experience: 8 years of recent relevant experience Skills and Knowledge: Comprehensive knowledge of diabetes care and management Understanding of adult learning principles and group dynamics Ability to develop collaborative educational approaches and curricula with a team Clinical assessment skills supporting autonomous practice in ambulatory settings Strong team leadership and organizational skills Additional Details Job Shift: Varied Schedule: Full Time Shift Hours: 8 hours Days of the Week: Monday - Friday Weekend Requirements: None Benefits: Yes Unions: No Position Status: Non-Exempt Weekly Hours: 40 Employee Status: Regular Sutter Health is an equal opportunity employer EOE/M/F/Disability/Veterans.
