Position Summary

The Youth Outreach Case Manager is responsible for providing enhanced case management services to young adults ages 18–24 who are experiencing significant barriers to stable housing, education, healthcare, and other critical needs. This role involves proactive outreach in the community, home visiting, and the development of individualized service plans that promote stability and self-sufficiency. The position includes regular travel within the community, and company vehicles are provided.

Essential Duties And Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Additional duties may be assigned that are essential to the role.

Provide intensive, strength-based case management to young adults aged 18–24 facing challenges such as homelessness, unemployment, mental or physical health issues, educational setbacks, and limited family support. Maintain an active caseload of young adults and conduct regular home and community-based visits to support goal achievement and stability. Conduct outreach and engagement efforts within the community to connect with youth in need of services. Develop individualized service plans in collaboration with clients, focusing on housing, education, employment, health access, and life skills development. Assist in the development of teen center programming and activities for school-age youth as needed. Facilitate small support groups and life skills workshops for youth. Coordinate referrals and service connections with internal programs and external community partners. Maintain accurate and timely documentation in compliance with agency and funder requirements. Monitor and track client progress through data collection, assessment tools, and regular reporting to program management and funders. Attend required training, supervision, and team meetings. Ensure client confidentiality and adhere to ethical and professional standards in all interactions.

Qualifications

Associate or bachelor's degree in social work, Child Development, Human Services, or related field preferred. Equivalent work experience will be considered. Experience working with youth, preferably in community-based or case management settings. Knowledge of local resources related to youth services, housing, education, health services, and employment. Strong interpersonal, organizational, and communication skills. Ability to work independently and as part of a multidisciplinary team. Must be able to travel locally for home visits and community outreach; company vehicle provided. Valid driver’s license and clean driving record required. Bilingual in English/Spanish a plus.

Language Ability

Must have high level language skills. Ability to read, analyze, and interpret general business periodicals, technical procedures, Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from managers, clients, customers, and the general public.

Reasoning Ability

Must have good reasoning skills. Ability to define problems, collect data, establish facts, and draw valid conclusions.

Computer Skills

To perform this job successfully, an individual should have good working knowledge of Windows operating systems, Microsoft Office Suite - Word, Excel, Outlook, and Internet browsers.
