Continuum Home Health Care is dedicated to providing quality in-home care that enhances the independence and well-being of seniors in our community. We’re growing and seeking a skilled Human Resources Coordinator to join our team and lead our HR functions with a focus on recruitment, employee engagement, compliance, and development.

Key Responsibilities:

Talent Acquisition & Retention:

Manage job postings, interviews, and the onboarding process to ensure a seamless experience for new hires.

Develop and implement strategies to reduce turnover and improve employee retention.

Employee Relations & Culture:

Act as the primary HR contact for employees, addressing questions and resolving workplace issues with empathy and confidentiality.

Foster a positive, inclusive, and supportive workplace culture through employee engagement initiatives.

Conduct regular check-ins, feedback sessions, and exit interviews to identify improvement opportunities.

Compliance & Policy Management:

Ensure compliance with state and federal labor laws, as well as home care regulations.

Maintain and update HR policies and procedures, keeping them aligned with best practices and industry standards.

Oversee accurate documentation and record-keeping, including employee files and payroll.

Performance & Development:

Implement performance management processes, including regular performance reviews and growth plans.

Create and support training programs that enhance skills and career development for all employees.

Provide guidance and coaching to managers on leadership and employee development practices.

Benefits Administration:

Oversee benefits administration, including health insurance, time off, and employee wellness programs.

Manage relationships with benefits vendors to provide competitive and cost-effective employee options.

Qualifications:

Bachelor’s degree in Human Resources, Business Administration, or a related field (HR certification preferred).

2+ years of experience in human resources, with a preference for experience in healthcare or home care settings.

Strong knowledge of HR compliance, labor laws, and best practices.

Excellent communication, conflict-resolution, and interpersonal skills.

Proficiency with HR software and tools (experience with Axis Care, Google Suite, Mondays, or Compass is a plus).

Ability to handle confidential matters with discretion and professionalism.
