This unique position supports primary care providers in improving patient outcomes through care coordination, quality gap closure, and health screenings. What You'll Do:

Support Preventive Care Initiatives: Help identify and close care gaps by reviewing patient charts and preparing for upcoming visits. Coordinate Screenings & Referrals: Use standing orders to place screening orders and manage in-network referrals. Outreach & Engagement: Contact patients to schedule important visits like annual wellness exams, well-child checks, and cancer screenings. Data Management: Transfer quality care data into population health platforms and work with reports to identify high-risk patients. Collaborate Across Teams: Work closely with primary care teams and clinical consultants to support value-based care efforts. Float Across Clinics: Be part of a dynamic schedulework from our Arcadia office on Mondays, visit clinics TuesdayThursday, and enjoy flexible Fridays (clinic or remote depending on needs). What You Bring:

Experience: 3+ years in a healthcare coordination role (Medical Assistant, Referral Coordinator, or Utilization Technician). Certification: CMA or diploma from an accredited Medical Assistant program. Tech Savvy: Comfortable with EHR systems and scheduling platforms. Communication Skills: Clear, professional verbal communication and a collaborative mindset. Organizational Strength: Ability to manage multiple tasks, analyze data, and meet goals efficiently.
