SUMMARY OF POSITION:
Under the general direction of the Population Health Manager/Lead, the Population Health Coordinator supports the planning, implementation, and coordination of population health initiatives across NEMS. This includes identifying and addressing care gaps, supporting patient outreach strategies, analyzing population health data, and collaborating with clinical and non-clinical teams to improve health outcomes for our patient populations.
The Population Health Coordinator will play a key role in promoting preventive care and chronic disease management efforts, improving performance on population health measures, and supporting team-based care models. This role requires strong communication and organizational skills to foster partnerships with healthcare providers, patients, and internal departments to ensure efficient, patient-centered care.
ESSENTIAL JOB FUNCTIONS:
Coordinates and supports population health projects and initiatives across NEMS. Conducts targeted outreach to patients to promote preventive screenings, chronic disease follow-up, and closing of care gaps. Monitors population health data and reports on performance against key metrics (e.g., cancer screenings, immunizations, diabetes control, hypertension). Collaborates with care teams, data analysts, and QI staff to identify trends, barriers to care, and opportunities for intervention. Prepares regular reports on population health outcomes and supports data-driven decision-making across departments. Works with clinical teams to implement and sustain workflows that improve population health outcomes and streamline outreach processes. Educates and engages patients on the importance of preventive care and chronic disease management, empowering them to participate in their own care. Supports interdepartmental coordination efforts for outreach campaigns and population health initiatives (e.g., FIT kit distribution, vaccine outreach). Participates in Performance Improvement/Continuous Quality Improvement activities. Performs other Establishes and maintains effective interpersonal relationships across all levels of staff and external partners. Works independently and as a collaborative team member. Communicates clearly and effectively, both verbally and in writing, with individuals from diverse backgrounds. Effectively uses computers, population health registries, and reporting tools to manage outreach lists, track interventions, and prepare reports. Coordinates with Information Systems as needed to support data integration and reporting needs. job duties as required by manager/supervisor. QUALIFICATIONS:
Bachelor's degree preferred or minimum of 1-3 years of work-related experience in population health, healthcare coordination, or community health. Experience with patient outreach, care gap closure, or healthcare quality initiatives preferred. Strong analytical skills with ability to interpret data and translate insights into action. Excellent customer service and interpersonal communication skills. LANGUAGE:
Must be able to fluently speak, read and write English. Fluent in Chinese (Cantonese and/or Mandarin), Spanish or other languages preferred Fluency in other languages are an asset. STATUS:
