Henry Ford Health dialysis facilities (Greenfield Health Systems) offer a compassionate, multi-specialty team, modern technology, clinical expertise, and the latest medical knowledge to provide renal patients the highest quality care available. We have been part of the community since 1983 and are close to celebrating almost 40 years of excellence dedicated to dialysis! Department Description
Northland Park Dialysis is a 32-station unit operating three shifts, six days a week handling complex patients from ages 20-90 years old. Many require assistance with mobility devices. Operating hours for patients are 5:00 am
8:00 pm and team members are from 4:45 am to 8:30 pm. Employee schedules are made through several shift designs. Patients are medically managed by several Nephrologists and Advanced Practice Practitioners (APP). We have Registered Dietitians and Social Workers to address patient needs such as transportation and dietary needs. Unit staffing includes a Nurse Manager, a Nursing Supervisor as well as many Registered Nurses and Dialysis Clinical Technicians. We have additional support staff of Biomed Techs and Dialysis Assistants overseen by a technical leader. What We Are Looking For
Masters-prepared Social Workers to work with our renal patients and families to assist with the adjustment to what is likely a life-changing event. Provide emotional and social adjustment counseling, assistance with removing barriers to treatment, education on kidney disease and treatment goals, and bridging barriers to referral services and community resources. Conducts initial psychosocial interviews with patients and families in order to assess baseline level of functioning, comprehension of illness and its ramifications, and impact of individual/family history upon current and future psychosocial functioning. Provides ego-supportive counseling to patients and their families in order to maximize ongoing psychosocial functioning. Provides crisis intervention to patients and families as needed. Functions as a patient advocate and as a liaison between the patient and multiple care delivery systems. Participates in comprehensive inter-disciplinary team patient care meetings with appropriate documentation on IDT assessment and plan of care. Records psychosocial assessments, update assessments, and progress notes in accordance with established recording guidelines. #IND2 Education/Experience Required
Masters of Social Work Degree required. LMSW (will consider LLMSW) Experience in a healthcare setting preferred. Demonstrates understanding of the impact of Chronic Kidney Disease (ESKD) on activities of daily living, quality of life, and mental health needs. Familiar with both clinical and case management techniques. Additional Information
