*Position Summary*
Under the general supervision of the Executive Director, the QIDP shall oversee the day to day operations of 2-4 ICF or Community Care Facility homes, responsible for case management of approximately 18 to 23 consumers, supervises House Service Coordinators (HSC) and staff assigned to designated homes, work as a liaison with multiple levels of staff and consultants, ensures all required reports, documents, and other mandated paperwork is submitted on a timely basis, participates as a member of the management team, Interdisciplinary Team (IDT), and Human Rights and Infection Control Committee, other duties as assigned by the Director of Training and Staff Development.
*Job Responsibilities:*
· Provide case management for consumers.
* Coordinate and compile ID team assessments within the first month of the client’s admission to TLC. * Complete monthly reports & prepare IPP documents for semi and annual IPP’s. * Ensure consistent and reliable data collection procedures across settings by performing reliability checks periodically, and train staff on data recording procedures for every domain until adequate reliability has been achieved. * Work with the nursing staff in coordinating medical, medication, and neurological consultation for each resident. Ensuring direct care staff are updated on developing issues regarding the consumer’s medical needs.
In partnership with the Lead HSC, manage and supervise HSC and Residential aides.
* Responsible for the group home’s staff schedules ensuring the home is operating per required ratios. Manages scheduling for all staffing needs and scheduling changes * Ensure required direct care staff training is provided on an ongoing basis. * Train staff on implementing goals and intervention techniques, to ensure that the procedure is consistently and systematically implemented across staff.
Work as a liaison with multi levels of staff, consumer’s family, consultants, and other outside entities.
* Participate as a member of Interdisciplinary Team. * Work with Interdisciplinary Team in the preparation and monitoring of Individual Program Plans for each resident. * Work with behavior specialist and/or other consultants to develop and implement appropriate programs for each client. * Conduct monthly update meetings with staff in order to keep IPP objectives current. * Organize training for consumer’s family/conservator on behavioral management and skill acquisition. * Report any incidents positive or negative to the consumer’s family/conservator. * Consult with related agencies and individuals as needed.
Complete and maintain required paperwork.
* Prepare incident reports and ensure that they are submitted to the appropriate parties (Residential Director, HR Director, and Nursing Supervisor) within 24 hours of the incident. * Responsible for household paperwork. * Manages staff’s time record keeping and submits Payroll.

*Qualifications:*
* Must qualify as a Qualified Intellectual Disabilities Professional (QIDP) under California ICF/DD-H regulations. * Bachelor’s degree in a social science preferred. * One year experience in administration of service programs for people with developmental disabilities. One year experience in community-based treatment services * Two years’ experience directing the work of others i.e., management or supervisorial experience. * Ability to set priorities and organize work responsibilities to ensure completion of assigned tasks within preset timeframes. * Must consistently be patience, tactful, and have a pleasant professional demeanor when communicating with all levels of staff, outside entities, and clients’ family members. * Demonstrate strong interpersonal, teamwork skills, leadership abilities, and a willingness to work harmoniously and professionally with different groups. * Ability to communicate and work effectively with people of diverse cultures, education, social, and economic backgrounds. * Proficient in computer operations with knowledge of Microsoft Office. * Ability to lift, push, pull, or move a 10-50 pounds. * Maintain a valid CA driver’s license with a driving record that is acceptable for TLC’s insurance carrier.
*PHYSICAL DEMANDS AND WORKING CONDITIONS:*
This position is primarily in a temperature controlled open office area. There is frequent sitting for long periods of time. Frequent interruptions and crisis intervention. Frequent typing on and viewing the computer, report writing, and charting. Frequent cell phone and land line telephone use. Frequent driving for monitoring TLC assigned homes and consumer’s day programs. This position can be moderately physical. There is frequent standing, bending, and fast-paced walking. Moderate interactions with consumers that are physically challenged that will need assistance. Assisting consumers in wheelchairs, with walkers, gait-belts that require pushing, supporting and guiding. On occasion, consumers may become physical including but not limited to hitting, kicking, grabbing, and biting. Ability to lift, push or move a minimum of 50 lbs.
*Occasionally, you are exposed to consumers’ bodily fluids which can include saliva (drool), blood, urine, fecal matter. Stringent Universal Health Precautions are practiced.*
Working conditions are subject to occasional weather elements, i.e. sun, wind and rain for outside activities.
