Description: This is a community-based hospital in an urban area. The facility has 304 licensed beds. The unit has 20 beds with an average daily census of 18. The patient population includes general medical patients needing advanced hemodynamics and continuous monitoring of cardiac, respiratory, hemodynamic, and neurologic status. Patients may experience respiratory compromise or failure requiring medium or high-flow oxygen devices, non-invasive positive pressure ventilation, or ventilator support. The unit also manages renal failure with continuous renal replacement therapy (CRRT) capabilities. Care is provided for severe sepsis with hemodynamic instability and IV resuscitation. The unit supports general, vascular, or thoracic surgical patients needing continuous cardiac, respiratory, or hemodynamic support, including pharmacological vasopressor support. Patients who experience cardiac arrest may require targeted temperature management. Neuro medical cases, including stroke with or without thrombolytic therapy and traumatic falls with neurosurgical intervention, are common. Patients with diabetic ketoacidosis or hyperglycemic hyperosmolar state may require insulin drips with titration. Patient ratios in the ICU are typically 1:2; high acuity cases may require 1:1 care. In overflow situations, the RN to patient ratio can be 1:3 or 1:4, depending on interventions. Support staff includes nurse tech support with charge nurses available for resource assistance. The required skills include expertise in titration of drips, ventilator management, and post-procedure site care. The unit features a rapid response team, availability of respiratory therapists, and has hospitalists and intensivists on-site 24/7. Technology utilized includes an electronic medical record system, IV pumps, and various monitoring and hemodynamics equipment. Floating may occur within the critical care division or intermediate care. The unit promotes a positive and team-focused culture, emphasizing collaboration among staff.
